Lloyd's algorithm needs to be applied before the mesh extrusion in order to keep identical starting and ending faces

merge patch from Jose Paulo Moitinho de Almeida for AlternateRight/AlternateLeft transfinite options

1) remove orientation: it's just wrong
2) remove any potential vertices belonging to GVertices from the list of vertices in a compound face in case of multiscale partitioning

instead of using the min edge length of tets in the initial mesh, we
 - use the max edge length of boundary triangles
 - and the min edge length of tets for the remaining vertices

Fopen deals with non-ASCII filename paths on Windows.

fix regression with extrusions of many surfaces: order faces by tag - this allows to minimize the number of passes necessary to resolve dependencies (otherwise 10 passes is not enough!)
