2D triangle does not match faces of any 3D tetrahedron
When generating 3D grids in domains with 2D surfaces as internal constraints, we quite often experience that the generated mesh has 2D triangles that do not match with any of the 3D tetrahedrons.
Illustration: The 3D cell (green edges) is pierced by a thin 2D face. The cell size is clearly too small to create a decent mesh, and that may be part of the problem, but my understanding is the mesh should still be conforming.
Attached is the .geo file, the .msh file generated by Gmsh 3.0.6, and the information printed by Gmsh. The troublesome cell is found on line 1291 in the .msh file (cell number 737) - there is no 3D cell that contains the same combination of nodes.

Also, thanks for an awesome program!
